Labor Guidelines on Vaccination of COVID-19 per Labor Advisory 3, Series of 2021 (PDF)
Labor Advisory 3, Series of 2021 was issued by the Department of Labor and Employment (DOLE) providing guidelines on the administration of COVID-19 vaccines in the workplace.
Essentially, the issuance mandates employers to adopt COVID-19 vaccination policy as part of their Occupational Safety and Health Standards (OSHS).
No costs of vaccine shall be passed on to employees, directly or indirectly. Likewise, employees cannot be compelled to take vaccine. Those who refused to get vaccinated should not be discriminated against.

Also, attached is a link to the draft/template of the Affidavit of Consent just in case one needs a reference for this purpose indicating consent to be vaccinated, waiver of liability (as to effects, etc.), and consent on use of personal information in relation to the Data Privacy Law.
