AWOL or Absence Without Official Leave is a ground for dismissal if it constitutes gross and habitual neglect of duty. Below is a Sample Notice of dismissal being the final step required to comply with the procedural due process of employee termination.

This refers to our show cause memo on the subject matter dated _______________. Our records reveal the following facts: You failed to report for work and was absent for a long period from ____ (date) ____ to ____ (date) ____, without official leave of absence.
On record, this is one of several instances of absences without leave which as of this date constitutes 11th instance and a total of twenty-four (24) days of unauthorized absences
Despite our show cause memo, you failed to submit a written explanation within the period provided. You also failed to appear at the scheduled conference on _________, which date also appears on the show cause memo.
In this connection, unauthorized absence or absences without leave (AWOL) constitutes gross and habitual neglect of duties, which is a ground for termination under the Labor Code. You were likewise invited to a hearing/conference however, you also failed to attend the same.
After a thorough review of the records and discussion on the matter, it is our view that your prolonged absence without leave is unjustified and a clear neglect of your duties in the Company.
In view of the foregoing, you are hereby informed that your employment with the Company is terminated effective on the closing hours of ______ (date).
Please comply with the relevant post-employment requirement of the Company by surrendering your accountabilities to _________________ (department or officer) for the Company to process your final salary. You are required to return all Company properties and documents to your Immediate Superior/ Human Resources not later than _______________ (date) or the cost of any unreturned company material or property may be charged to your account.
